Position Summary:

The Registered Nurse is responsible for providing specialized nursing services to patients and caregivers at home or in alternative settings. Provides technical nursing and skilled interventions as ordered by the physician. Instructs motivates and assists patients / caregivers in learning and improving ability to live independently at home.

Essential Functions and Job Responsibilities:

• Makes initial visits, processes orders, notifies physician of patient needs and changes in condition.
• Initiates care completes certification/recertification orders, and discharge summaries, if required.
• Regularly re-evaluates needs of the patients.
• Counsels with the patient and family/significant others on nursing, teaching, and related needs.
• Informs the physician and personnel of changes in the condition and needs of the patient.
• Consistently demonstrates ability to adequately complete all documentation and charting procedures in compliance with ACHC and company policy and procedures.
• Reviews Plan of Treatments and Care Plans to assure they are accurate and up to date.
• Documents procedures including how the patient tolerated a procedure, side effects and other pertinent information.
• Assists with authorization for CPAP/BiPAP referrals for patients. 
• Shows adequate knowledge of respiratory equipment and displays ability to utilize knowledge in the clinical setting.
• Demonstrates ability to effectively troubleshoot equipment in the field successfully.
• Displays knowledge of assessment skills and demonstrates application of clinical skills during set-ups, follow-ups, and in-services.
• Participates in discharge planning of highly technical cases.
• Performs clinical assessments as needed and reports results and recommendations to the referral and physician.
• Participates in highly technical discharges and prepares in advance to assure the patient and caregivers have a smooth transition to the home setting.
• Performs in-services to hospital staff, referrals, other professionals regarding equipment & issues of clinical nature.
• Follows up with physician and referrals regarding patient status and documents accurately and in a timely manner.
• Retain knowledge of and consistently adhere to procedures for the use of Personal Protective Equipment (PPE), infection control and hazardous materials handling. 
• Works to promote AdaptHealth by new program development, operational backup, personal visits, coordination of educational activities, etc.
• Assume on-call responsibilities during non-business hours in accordance with company policy.
• Prepares clinical records, care plans, progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner per company policy.
• Provides case management to enhance patient outcomes
• Identifies nursing diagnosis/patient care problems and interventions making additions/changes to ensure the patient’s problems are comprehensively addressed to avoid rehospitalization.
• Initiates and or coordinates the plan of care to include other disciplines as needed and act as a case manager to ensure integration of care is provided.
• Assesses the patient’s environment with particular attention to safety to ensure the environment is suitable for the delivery of safe and effective care
• Participates in in-services programs and presents in-service programs
• Conducts patient care conferences on patients assigned to his/her care.
• Attends all patient care conferences and meetings as scheduled.
• Participates in peer review and performance improvement as assigned.
• Participates in review of clinical records as assigned.
• Assume on-call responsibilities during non-business hours in accordance with company policy.
• Maintain patient confidentiality and function within the guidelines of HIPAA.
• Complete assigned compliance training and other educational programs as required.
• Maintain compliant with AdaptHealth’s Compliance Program.
• Perform other related duties as assigned.
