Muskegon to Grand Rapids

Full-Time | Monday–Friday, 8a–5p | $5,000 Sign-On Bonus!

🌼 What You’ll Do

• Perform thorough patient assessments and develop individualized care plans

• Coordinate with an interdisciplinary team to deliver high-quality hospice care

• Educate and support patients, families, and caregivers on hospice philosophy, pain control, and symptom management

• Monitor changes in patient condition and communicate updates promptly

• Provide emotional and spiritual support during end-of-life care

• Serve patients in home settings, nursing facilities, and assisted living communities

🩺 What You’ll Need

• Graduate of an accredited nursing program (Diploma, ADN, or BSN)

• Current RN license in the state of practice

• 1+ year of clinical experience (hospice or home health preferred)

• Reliable transportation, valid driver’s license, and proof of auto insurance

• Ability to lift, move, and assist patients (up to 50–100 lbs)
