Are you a passionate and skilled Occupational Therapist looking to make a meaningful impact in your community? As an Occupational Therapist, you will provide in-home evaluations, treatments, and education to help patients improve their ability to safely and independently perform daily activities. You will work closely with nurses, physical therapists, speech therapists, and other members of the interdisciplinary team to ensure comprehensive, patient-centered care.

Duties to Include:



Conduct comprehensive in-home occupational therapy evaluations


Develop and implement individualized treatment plans focused on improving activities of daily living (ADLs) and instrumental activities of daily living (IADLs)


Provide therapeutic interventions to improve upper extremity strength, coordination, cognitive skills, and functional independence


Train patients in adaptive techniques to enhance safety and independence with bathing, dressing, grooming, toileting, and meal preparation


Recommend and educate on adaptive equipment and home modifications to improve safety and accessibility


Educate patients and caregivers on home programs, energy conservation techniques, fall prevention strategies, and equipment use


Collaborate with nurses, physical therapists, speech therapists, social workers, and case managers to coordinate care


Document all patient visits, progress notes, and care plans in compliance with agency and regulatory standards


Monitor patient progress and adjust treatment plans as needed


Communicate changes in condition or concerns to the interdisciplinary care team


Promote independence and ongoing improvement through compassionate, patient-centered care

Qualifications



Licensed to practice as an Occupational Therapist in the state of Iowa.


Minimum of two years of experience in occupational therapy; community/home health experience preferred.


Strong communication, organizational, and problem-solving skills.


Valid driver’s license with reliable transportation.
