Registered Nurse- Hospice
Reports to: Assistant Manager
Required:
- Current NJ RN License
- BLS
- Associates Degree in Nursing
- Driver's license in good standing
Preferred:
- Minimum of one-year experience in acute, subacute or home/hospice care setting
- Experience using an EMR, excellent critical thinking and communication skills.
- BSN
Duties & Responsibilities:
Performs Patient assessments:
- Accurately assesses patient’s appropriateness for hospice care, learning needs and progress toward goals
- Develops the plan of care for the patient and family.
Ensures appropriate utilization of services:
- Maintains visit productivity standards
- Demonstrates knowledge of insurance reimbursement.
Completes discharge assessments:
- Performs laptop transfer of data twice daily
- Completes admission assessment and transfers within 24 hours
- Completes re-visit documentation and transfers.
Uses age appropriate teaching methods for patients and families:
- Evaluates teaching plan and revises as necessary
- Documents patient education tools used.
Assumes responsibility for Case Management:
- Develops/implements/evaluates plan of care based on evidence-based knowledge
- Revises care plan as indicated
- Prioritizes care needs
- Demonstrates knowledge of hospice regulations and insurance reimbursement guidelines
- Shows evidence of interdisciplinary communication
- Performs HHA supervisions per standards
- Ensures appropriate utilization of services
- Follows scheduling process
- Communicates progress toward goals with IDT
- Evaluates the plan of care and revises as needed
- Prioritizes schedule to accommodate patient's needs
- Documents case management activities
- Demonstrates knowledge of pharmacology and safe medication management
- Routinely reconciles medications. Maintain current medication orders.
Maintains safe environment for patients and co-workers:
- Follows infection control measures and reports infections
- Demonstrates knowledge of pharmacology and safe medication management
- Routinely reconciles medications
- Maintains current medication orders.
Attends monthly staff meeting:
- Participates in PI meetings and activities/committees as requested.
- Accepts other assignments as requested.
Participates in Performance Improvement Program:
- Reviews health records and gathers data to support PI activities
- Attends UR meetings as requested
- Provides orientation/ mentorship to new employees
- Cross trains to learn new skills and demonstrates a willingness to teach other.
Assists with growing agency business:
- Provides orientation to new employees
- Accepts other assignments as requested
