Job Title: RN MDS Nurse (Registered Nurse – Minimum Data Set Coordinator)

Position Summary:
We are seeking a detail-oriented and experienced RN MDS Nurse to join our team. This role is responsible for coordinating and completing the Minimum Data Set (MDS) assessments in accordance with federal and state regulations. The ideal candidate will ensure accurate resident assessments, care planning, and compliance while collaborating with interdisciplinary teams to deliver high-quality patient care.

Key Responsibilities:

• Complete and oversee MDS assessments (admission, quarterly, annual, and significant change).

• Ensure compliance with CMS guidelines and state/federal regulations.

• Coordinate care planning with interdisciplinary team members.

• Review medical records for accuracy and completeness.

• Monitor and support quality measures and reimbursement optimization.

• Participate in audits and implement corrective action plans as needed.

• Educate staff on documentation and MDS-related processes.

Qualifications:

• Active Registered Nurse (RN) license in the state of Ohio.

• Previous experience in MDS coordination preferred.

• Strong knowledge of CMS guidelines, RAI process, and care planning.

• Excellent attention to detail and organizational skills.

• Ability to work collaboratively with clinical and administrative teams.

• Proficiency in electronic health records (EHR) systems.

Work Schedule:
