Job Description

Basic Purpose

Positions in this job family are assigned responsibilities involving the supervision and management of a staff, program, unit, facility, or statewide geographical area that provides direct or indirect health services. This includes responsibility for broad management issues in planning, organizing and integrating various functions for which responsibility is assigned and directing assigned staff in meeting the goals and objectives of the program.


Typical Functions

The functions performed in this job family will vary by level, unit and organization, but may include the following:

  • Assist MDS personnel in completing MDS and Care Plans. Ensure new MDS/Care plans are initiated and completed on new admissions, current residents who are due quarterly reviews, annuals and significant change within the time frame required by RAI/MDS 3.0. Perform interviews and assessments weekly for residents and document accurately in PCC. Gather data using PCC documentation and contribute to on-going assessments. Complete Initial, Annual, significant change, and Quarterly MDS assessments in Point Click Care under supervision of DON and/or ADON. Assist with calendar and IDCP agenda for listings of residents who are due an initial, annual, quarterly and/or significant change and distribute agenda to all disciplines. Evaluate all residents re-admitted from hospital for change of status. Post a 7-day assessment list of residents on the IDCP agenda. Attend weekly IDCP meeting with other disciplines to discuss resident’s plan of care. Report any problems with other departments concerning MDS assessment completion to DON and/or ADON promptly.
  • Stay updated on all MDS changes and regulations by attending MDS 3.0 updates and in-services keeping administration, supervisor, and other staff members current on any changes. Attend all required meetings regarding resident changes.
  • Complete 30-day review on all residents with restraints and present in IDCP on the (1st) Thursday of each week. Care Plan updates on all residents initially, quarterly, and annually.
  • Supervise PCAs who participate in IDCP. Evaluate employee performance within designated time frame through periodic and annual reviews. Initiate steps of corrective discipline when necessary. Assess staff training needs and provide 1:1 training and/or alert In-Service educator.
  • Follows, interprets, and enforces policies and procedures of this facility. Participate in assessing and planning care of residents including behavior management goals and interventions.
  • Perform Registered Nurse duties on units as required by staffing as available.
  • Plans, organizes and directs health services for an assigned program, unit, facility, hospital or geographical area; ensures that goals, objectives and work performed are consistent with standards and that quality care is provided to patients and clients. Selects, mentors, motivates and directs development and evaluation of the staff; establishes and tracks quality improvement indicators and other information concerning services and care to ensure that resources are available to provide quality services and on-time performance consistent with standards; may devise health related educational programs for professional and non-professional staff. Participates, consults and collaborates in the development of long-range plans for health care programs; may interpret and communicate agency and departmental policies and procedures; establishes areas of responsibility and assignment; develops or implements policies and procedures to insure efficient and effective delivery of health services.
  • Receives, analyzes and evaluates reports and other information and coordinates quality improvement activities concerning the care of patients, equipment, environmental health and safety conditions, and other related services in improving the quality and utilization of health care. Develops budget estimates or other information concerning financial requirements; approves or monitors expenditures, purchases and other actions to ensure compliance with budget guidelines and effective delivery of services.
