Are you an experienced Registered Nurse passionate about public health and making a meaningful impact? The Chesapeake Health Department is seeking a dynamic Senior Public Health Nurse (PHN Sr.) to lead and support the Long-Term Support Services (LTSS) program, while playing a vital role in communicable disease control and prevention. The incumbent will serve as an expert and leader in managing the LTSS program, guiding our response to critical public health issues like elevated blood lead levels in children and human rabies prevention. This position works with a multidisciplinary team, including social workers and community partners, to improve health outcomes and support vulnerable populations. Primary duties include conducting joint home visit- LTSS screenings with Social Workers from the Department of Social Services, to assess adult clients who may be at risk of nursing home placement, and to identify appropriate support or referral services. For pediatric clients, LTSS screenings are performed independently to evaluate needs and coordinate care. Thid PHN Sr. position is also responsible for the public health nursing response to cases involving elevated blood lead levels in children and will provide expert nursing interventions for human rabies prevention. Other duties of the position involve supporting community-based prevention efforts, participating in outreach and education initiatives, working specialized vaccination events, and serving as a key member of the district’s Epidemiological Response Team.

In the event of a Governor's emergency declaration, public health nurses are required to assume additional duties for the district's emergency preparedness and response (EP&R) activities. This includes, but is not limited to, point-of-dispensing (POD) mass vaccination events and shelter coverage. During declared emergencies, the health department will adjust work hours, locations, job duties, and participation in training to meet public health and business needs. Additional potential work-schedule variations include involvement in community outreach
events and other EP&R assignments, such as communicable disease investigations, epidemiological events, and city emergency shelter duties. Actions performed at this level require licensure as a Registered Nurse by State code.

This position offers up to a $3,000.00 hiring incentive.

Minimum Qualifications

Must be certified in Basic Life Support (BLS)/CPR, able to be fit tested and perform fit testing for other staff in accordance with OSHA Respiratory Protection Standard (29 CFR 1910.134) [Ref.2] regulations

Experience functioning in a leadership role; experience and ability to function independently in the home/clinic/community/school settings and prioritize workload

Exercises sound nursing judgment; effective verbal, written, interpersonal/physical assessment skills

Experience and ability to enter, review, analyze data and statistics and produce reports and/or recommendations from such information; ability to work in a fast paced, data driven environment

Experience and knowledge of nursing process, health maintenance, disease processes, physical and behavioral sciences, case management process and basic principles of epidemiology

Work effectively as a member of the multidisciplinary health care team

Knowledge and experience in use of computers and computer applications; including Internet, Microsoft Office, and Google Mail applications

Experience and ability to work with diverse cultural backgrounds

May travel and some weekend and/or evening hours may be required

Excellent communication, data interpretation and computer skills

Must be able to demonstrate decision making skills, teaching, interviewing, counseling skills and the ability to analyze problems/situations and problem solve
