$81.330 - $104.950 - $128.560
The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.
Full Time position
8-hour shift between 8:00am - 6:00pm
Weekend shifts as needed
What You Will Do
This role will be working in an ambulatory care setting providing one on one patient consultations and follow up.
Required Qualifications
- Other : Education pursuant to licensure requirements.
- Must meet two of the following three requirements: One year accredited residency in a hospital pharmacy or ambulatory care; Board of Pharmacy Specialties (BPS) Certification; or at least three years’ experience directly providing or preferably leading collaborative drug therapy management (CDTM, aka “pharmacy consult”) services.
- AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association -REQUIRED
- California Pharmacist License - CA State Board of Pharmacy -REQUIRED
Other Qualification Requirements
- Immunization Certification. If considered and no residency, must obtain relevant Board certification within 1-2 years, defined by hiring manager.
Preferred Qualifications:
- BCACP certification via Board of Pharmacy Specialties (BPS)
- Experience with Epic
Essential Functions
- Clinical and educational services
Has met minimum certification/recertification requirements required for hospital CDTM services offered at entity as outlined in policies and procedures.
Participates in Clinical Services for example, CDTM, treatment team participation, and other services specific to the job.
Demonstrates knowledge of consult services as evidenced by attainment of consult service goals and proper documentation of actions, recommendations, and patient outcomes on the pharmacy monitoring forms.
Performs by leaving concise, objective, legible, and professional notes in the patient's clinical chart or via clinical intervention notes.
Consistently demonstrates ability to complete formal clinical workload assigned during shift as evidenced by completing all consults and advisory services in a timely manner.
Participates in the selection, development, implementation, outcomes analysis, reporting, and maintenance of selected pharmacotherapy improvement programs, including disease state management programs, or other cross-continuum pharmacotherapy programs. Reviews and reports on progress of integration plan and identify additional areas and plans for improvement.
Provides and / or coordinates formal needs-based education. Educates, promotes and assists the coordination of staff involvement in clinical responsibilities with formal certification in key activities and skill sets.
Coordinates and provides educational in services and information presentation to hospital and medical staff committees and the medical, nursing or pharmacy staffs, students, residents, and/or patients. Provides staff development through newsletter articles, drug monographs, and mentoring.
Participates in meeting preparation for the P&T and SMP Committees. Participates in medical staff advisory groups, multidisciplinary teams, focus groups, task forces, or other designated activities that will further the practice of pharmacy.
Provide patient education on medication use and disease states.
Collaborate with other members of the health care team and the pharmacy staff to recommend/review current and new agents/ protocols (i.e. formulary status, usage guidelines, and care maps). - Clinical consult services
Provides CDTM services in compliance with P&T approved SHC CDTM protocols.
Reviews patient medication therapies for appropriateness of drug therapy, therapeutic duplications, allergies, demographics, minimum and maximum dosing guidelines, special population requirements, medication history, non-pharmacologic therapies, socioeconomic, and other relevant factors.
