We are among the nation’s Best Places to Work and pride ourselves on our compassionate patient centered care and team-oriented culture.  Shift Information:This position is Full Time working three 12 hour days (Saturday, Sunday and one other week day, 8am-8pm)Currently offering a $15,000 sign on bonus for qualifying full time RNs. What we offer:Health Insurance401(k)Dental InsuranceVision InsurancePaid time offTuition Reimbursement | HPSA DesignatedEmployee Education ScholarshipsEmployee Referral ProgramMerit IncreasesFree access to a beautiful onsite fitness facility What you will do:The Home Care Weekend RN is a professional nurse who coordinates and directs the home care patient's care in their home environment (private residence, assisted living, etc.) based on individual needs.  The Home Care Weekend RN is responsible for independent management of the Home Care patient population requiring use of advanced assessment, teaching, and decision making skills. The Home Care Weekend RN completes admission/start of care visits, resumption of care visits, and routine visits.  The Home Care Weekend RN evaluates the patient for appropriateness of home care needs and develops the plan of care in collaboration with the provider, caregivers, and Home Care IDG members. The Home Care Weekend RN provides patient care based on provider’s orders, departmental policies and procedures, regulatory guidelines, and best practice. The Home Care Weekend RN documents in the patient’s medical record in a timely manner according to agency policy.  The Home Care RN Case Manager performs on-call responsibilities including triaging patient calls and providing prn visits as needed.  The Home Care Weekend RN also assists with back office tasks as appropriate. Required Education: Graduate of an accredited Diploma of Nursing Program or Associate’s Degree in Nursing.
Preferred Education:
Bachelor’s Degree in Nursing

Required Skills:
Nursing skills as defined by generally accepted standards of practice. Basic knowledge and skills to care for the clinical needs of the population served. Professional and efficient verbal and written communication skills. Professional interpersonal skills. Ability to multi-task, basic organizational and time management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (i.e., copy machine, fax machine, etc.) Must have demonstrated experience in working with patients and caregivers dealing with acute and chronic illness. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.

Preferred Skills:
Experience with Home Care Home Base; Experience with smart phones and smart devices

Required Licensure/Registration/Certification:
Current Texas state license in appropriate discipline; Current American Heart Association CPR; Current Texas state driver’s license; Current auto insurance. Must have reliable transportation.

Preferred Licensure/Registration/Certification:
OASIS certified; Wound Care certified; Advanced education or certification in area of specialty

Required Experience:
One year experience in a healthcare setting in respective discipline

Preferred Experience:
Medical-Surgical experience; Experience in Case Management; Supervisory position in healthcare setting; Home Care of Hospice experience
