Duke University Hospital is seeking a dedicated Outpatient Clinical Nurse to serve as a Clinical Coordinator and Nurse Triage Specialist for the Transplant Infectious Diseases Outpatient Parenteral Antimicrobial Therapy (TID OPAT) program. This role supports safe, coordinated, and high-quality outpatient antimicrobial care for complex immunocompromised patients.

 

Department Profile: 

The Transplant Infectious Diseases subsection within the Division of Infectious Diseases specializes in caring for immunocompromised patients with highly complex infections—including individuals with solid organ transplants, hematopoietic cell transplants, hematologic malignancies, recipients of cellular therapies, and ventricular assist and other advanced cardiac support devices.

The newly established TID OPAT program enhances the care of patients requiring outpatient intravenous antibiotics or high-risk oral antimicrobials, providing structured monitoring and seamless care transitions.

Work Schedule:

Outpatient, On-site

Full time: (35-40 hours/week), Monday to Friday

No weekends or holidays

 

Key Responsibilities:

  • Collaborate with the OPAT team (PharmD, Medical Director, Transplant ID providers, administrative staff) to ensure safe and coordinated care for patients receiving IV antibiotics.
  • Monitor, analyze, and document weekly therapeutic drug levels, safety labs, and inflammatory markers; escalate abnormal results to the PharmD or providers as needed.
  • Provide telephone triage support to determine the need for urgent or emergent evaluation (e.g., vascular access device education, assess vascular access device malfunction, infection progression, adverse drug reactions, acute kidney injury).
  • Manage incoming calls and faxes from the dedicated OPAT phone line to address patient needs promptly.
  • Coordinate care among ID providers, home health agencies, and infusion partners to ensure seamless transitions and prevent avoidable ED visits and hospital readmissions.
  • Triage urgent inpatient and outpatient referrals to Transplant Infectious Diseases.
