Overview

This position reports to the Regional Head of Clinical Field Operations, the Nurse Practitioner Lead is responsible for recruitment, training and management of Registered Nurses (RNs), Nurse Practitioners (NPs), Licensed Clinical Social Workers (LCSWs), and Physician Assistants (PAs), ensuring effective collaboration, optimal staffing, and high-quality patient care delivery to Care at Home family members transitioning to home health care. This role involves strategic planning, team leadership, performance monitoring, in partnership with the Community Care Manager, overseeing an interdisciplinary team to meet organizational goals, improve member outcomes, and provide exceptional patient care. The Nurse Practitioner Lead will also develop processes and policies to ensure members, and their families, receive timely and effective navigation support, both telephonically and in the field. The Nurse Practitioner Lead will have a strong knowledge base in providing care in the home, Value Based Care, HEDIS and Stars, documentation and coding, and medical practice principles. They will stay informed about advancements in healthcare practices and disseminate relevant knowledge.

Duties & Responsibilities

  • · Manage clinical team members, providing mentorship and guidance, regular performance evaluations, constructive feedback, and supporting professional development.

    · Execute strategies and solutions to create clinical and business value for the member populations and the provider practices within their region.

    · Ensure staffing levels and schedules optimize productivity, meeting patient care needs, maintaining high standards of care and safety in compliance with regulatory requirements.

    · Oversee daily operations to ensure compliance with organizational policies and procedures.

    · Monitor and evaluate patient care delivery to maintain high standards of quality and safety.

    · Implement evidence-based practices and clinical protocols.

    · Address and resolve patient or staff concerns promptly and professionally.

    · Coordinate orientation and continuing education for clinical staff, promoting opportunities for professional growth.

    · Serve as a liaison between clinical staff and administrative leadership.

    · Participate in interdisciplinary team meetings to enhance patient care coordination.

    · Ensure all staff adhere to regulatory standards, including HIPAA and OSHA requirements.

    · Provide back up call to an overnight, holiday and weekend nurse line for clinical escalations in multiple states.

    · Licensing requirement:

    · Willingness to obtain and maintain Nurse Practitioner licensure in multiple states to support on-call backup coverage, with all associated licensing costs covered by the company.

    · Maintain accurate documentation of clinical activities, training, and compliance records.

    · Prepare and present reports on team performance and operational metrics.

Qualifications

  • · Education:

    o Master’s degree in nursing or related science field required.

    o Certification/Licensure: Active and unrestricted NP or PA license required.

    · Experience:

    o Clinical: A minimum of 5+ years of strong leadership and management experience in a clinical setting; community-based care experience preferred.

    o Clinical: Must have 5+ years of clinical leadership experience; community experience preferred.

    o Experience with Medicare/Medicaid is a must.

    o Experience with implementing and maintaining regulatory requirements for clinical staff including NPs, PAs and RNs.
