Job Description:-
Clinic Care Coordination: Assists the clinician in delivering patient care includingtaking vital signs, preparing immunizations, Dressing changes, pulse oximetryand respiratory treatments and other required procedures. Accurately andefficiently documents findings, plans and interventions in the medical record.Evaluates the effectiveness of care and documents subsequent changes to planor follow up on medical record.

Plan of Care: Assesses individual physical, psychosocial and developmentalneeds of patients. Preprocesses and post processes the patient chart for eachclinic visit. Establishes or reviews plan of care and modifies as needed based onassessment. Implements the established plan of care competently andconsistently. Demonstrates advanced clinical and technical skills and canaccurately assess and intervene when deviations from expected outcomes occur.

Provides Patient and Family Education: Identifies specific education needs forpatients/families. Accesses and utilizes appropriate teaching tools andtechniques to facilitate patient/family education. Utilizes appropriate style andlevel of communication. Evaluates and documents effectiveness of teaching plan.

Documentation: Responsible for documenting contacts, chart notes and otherrequired data in medical record and the pertinent information systems used byUF CARES including Immunization Databases, Provide Case Management CMS,Specialty SNAQ Reports, Epic and other systems as required.

Monitors the quality of practice within HIV infected/exposed patient population byparticipating in continuous quality improvement program.

Participates as a member of the healthcare team and acts as liaison betweenpatients, families and the clinical team to ensure quality customer service. Actsas a resource person for peers and inpatient staff and demonstrates clinicalexpertise for specific population. Attends pertinent educational activities andreviews current literature relevant to clinical area of expertise.

Under Universal Precautions performs phlebotomy, HIV testing and otherassigned labs as necessary.

Recognizes the need for referral to other sub specialists, other agencies forproviders and facilitates the process. Coordinates with outside pharmacies,patient hospital discharge planning and specialists as necessary for patient care.

Assures continuity of care by collaborating with other health care providers,community agencies and other institutions. Participates in multi-disciplinarystaffing, discharge staffing, family conferences, etc.
- Participates in relevant nursing and specialty organizations.

Attends and participates in research, department and hospital-wide meetings.Some minimum travel between clinics.
Expected Salary:
Salary commensurate with education and experience.
Minimum Requirements:Graduate of a nationally accredited nursing program and one year of professional nursing experience. Licensed as a registered nurse in accordance with Chapter 464, Florida Statutes.
Preferred Qualifications:- HIV Experience preferred.
- Outpatient clinical experience preferred.
- Able to perform phlebotomy.
