About the role

The Team:

Mersey Leven Community Nursing and Home Care Service (MLCHN&HCS) operates within a Primary Health framework to deliver nursing and home care services to clients within our catchment area. Our catchment area covers from Ulverstone in the west, to Baker Beach in the East and South to Lorinna and Moina. Clients also receive care in Community Nursing clinics in Devonport and Ulverstone by appointment only.   The Community Nursing Services operating from MLCHN&HCS include the following: • Nursing • Hospital in the Home • Palliative Care • Personal Care • Domestic Assistance • Home Maintenance   The primary goal of the Community Services is to assist with the below: • Facilitate hospital avoidance • Facilitate discharge from hospital • Support people at risk to live at home for as long as possible   The Mersey- Leven Community Nursing (MLCHN) delivery models include: • Community Nursing • Community Nursing Enhanced Care Service (CoNECS) • Hospital in the Home (HITH) programme   Nursing care provided by MLCHN includes but is not limited to: • Chronic and complex wound care including Negative Pressure Wound Therapy • Management of venous access devices (PIVC & CVAD) • Indwelling and suprapubic catheter management • End of life palliative care including syringe driver management • Post-acute care following discharge from hospital • Medication management including support of medication administration via various routes and devices.

The Role:

Mersey Leven Community Nursing and Home Care Services is currently seeking applications for a passionate and experienced Registered Nurse to join our dedicated team. 

The Registered Nurse – Community:

Consistent with ongoing experience strengthens health outcomes through the provision of safe quality, clinically appropriate nursing care in partnership with patients/clients, their families and other health professionals within the defined community practice area. 

You will need: 

  • Communication & Collaboration: Excellent interpersonal skills and the ability to coordinate care effectively with hospitals, GPs, allied health professionals, and clients/families/carers
  • Autonomy & Teamwork: A proven ability to work independently as well as within a multidisciplinary team environment.
  • Commitment to Learning: A passion for promoting a learning culture through education and clinical decision-making support, along with a dedication to ongoing professional development.

Details of appointment:

  • Permanent, part time, shift worker position, working up to 64 hours per fortnight, commencing from as soon as you can join us!

*notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $80,524 to $109,175 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.

Access to the Grade 4 salary range $102,439- $109,175 is subject to successful application for progression to Grade 4

In addition, the following allowances calculated on the salaried incremental point may be available:

  • Post Graduate Allowance of up to 10 % based on qualification type
