Role Description 

The Community Partnerships Manager is responsible for developing and sustaining strategic relationships with local organizations in the California Bay Area and the Central Valley that serve our addressable population. This role is focused on establishing a strong referral network by creating mutually beneficial partnerships with hospitals, community-based organizations, shelters, and other service providers. The ideal candidate is highly relational, organized, and mission-driven, with a deep understanding of the local community and a track record of stakeholder engagement. 

This is a newly created role at Zócalo Health. The ideal candidate is excited by the opportunity to build from the ground up. You must be comfortable operating in ambiguity, creating structure where none exists, and iterating rapidly as we grow. We are looking for a self-starter who can implement new processes, help define best practices, and scale our approach in a fast-moving, mission-driven environment.

This position will report to the Regional General Manager of the Bay Area. 

 

The Community Partnerships Manager, Bay Area will contribute in the following ways:

  • Identify, develop, and manage relationships with key patient referral sources, including local hospitals, community based organizations (CBOs), shelters, clinics, and other relevant institutions.
  • Set and track partnership goals related to referral volume, partner engagement, and conversion rates; proactively adjust strategies based on performance data.
  • Educate partner organizations on Zócalo’s services and build clear, actionable referral pathways.
  • Manage a portfolio of community partners, ensuring we have regular check-ins and ongoing partner engagement.
  • Coordinate with the Community Engagement team to plan and participate in partner events, co-hosted initiatives, and collaborative outreach efforts.
  • Serve as the internal point of contact for partner-related insights and needs.
  • Collaborate with the Community Engagement team to align on shared goals and optimize referral-to-enrollment pathways.
  • Track, analyze, and report on referral volume and partner engagement using internal systems and tools.
  • Act as a community representative for the organization, embodying its mission and values.
  • Participate in regional operating committee meetings as a key stakeholder. 

Qualifications

  • 5+ years of experience in community partnerships, outreach, or stakeholder engagement.
  • Strong knowledge of local health, housing, and social service networks.
  • Comfortable working in goal-oriented environments with weekly, monthly, and quarterly performance expec
