POSITION TITLE:  Assistant Director of Nursing

RESPONSIBLE TO: DIRECTOR OF NURSING

EXPOSURE LEVEL:  CATEGORY II

Summary:

Assists the Director of Nursing in planning, organizing, developing and directing the day to day functions of the Health Services including: Nursing , Medical Records,                            and Transitional Care in accordance with current federal, state and local standards governing long-term care facilities, and as directed by the Director of Nursing, to ensure that the highest degree of quality care can be provided to the residents at all times.

ESSENTIAL JOB FUNCTION:  Responsibilities and Duties

  • Assists the Director of Nursing in      planning, developing, organizing, implementing, evaluating, and directing      the day to day functions of the Health Services, its programs and      activities
  • Apply knowledge of nursing      and informatics to assist in the design, development, and ongoing      modification of computerized health care systems.
  • Educates staff and assists in      problem solving to promote the implementation of the Electronic Medical      Record. 
  • Assists Nursing Administration team in      changing the culture related to implementation of the Electronic Medical      Record. 
  • Provides continued staff training on      technology.
  • Attends all cottage care conferences.
  • Assists in developing and implementing      methods for coordinating all Health Services.
  • Participates in developing,      implementing, maintaining and periodically updating written policies,      procedures, consistent with the Mission and Philosophies of St. John's      United. 
  • Participates in surveys and inspections      by government agencies and make reports and recommendations to the Director      of Nursing and Senior Administrative Officer concerning operations of the      department.
  • Developing relationships by welcoming      residents and families, friends, and new staff to St. John's United.

· Ensure any unusual occurrences or accidents are reported immediately to the Director of Nursing.

· Report any Health Services Educational needs to Director of Nursing.

· Nursing Care Functions when assigned.

· Ensures excellence of care by enhancing continual quality assurance by daily contact through reporting, and communication with professional staff.

· Provide leadership and management in the skilled nursing environment to

minimize infections and ensure staff is following proper precautions to prevent the spread of infection and incorporating appropriate measures to minimize exposure to infections. Incorporate advanced analysis, organizational, and problem solving skills to effectively manage infections for all staff and residents.

ESSENTIAL JOB FUNCTION: MDS

  • To manage      NF/SNF residents and the RAI process in accordance with current Federal,      State, and Local standards governing long-term care facilities, and as may      be directed by the Director of Nursing to ensure that the highest degree      quality care can be always provided to the residents.
  • Assess and review      charts of NF/SNF residents.
  • Lead team to      assure real time participation in interdisciplinary care plan process.
  • Participate in      interdisciplinary care plan meetings with interdisciplinary team per care      schedule providing input regarding resident care needs and goals.
  • Review NF/SNF      residents with appropriate Medicare/Insurance reviewers for current and      previous residents.
  • Consult with      Business Office regarding assessment times and payment status. Assist      Business Office with ICD-10 coding.
  • Complete      comprehensive MDS and Quarterly assessments for weekly scheduled      residents.
  • Oversees      Restorative program including Care planning, monthly, and quarterly      meeting notes.
  • Coordinate RAI      process with department.
  • Complete CAA’s      as assigned.
  • Complete baseline      care plan summary for new admissions.
  • Helps IDT team      update care plans quarterly/annually.
  • Review/Audit      completion of MDS, CAA’s Care Plans, lock and print as required.
  • Transmit MDS      assessments to State weekly.
  • Be familiar      with State and Federal regulations to maintain survey compliance. 
  • Be “On-Call”      on a rotation with nursing administration,
