THE ROLE:
The Social Requisition Specialist is responsible for operational functions of the Social Recreation service line for our Disability Services Division to drive program growth and maximize impact to the people we serve. The Social Recreation Specialist will work closely with other key operational departments to ensure smooth service delivery from referral to ongoing service delivery. Embodies and fosters the Company culture by upholding Company core values and striving to make an impact by making a difference every day.
Job Responsibilities include:

Conduct assessments, develop Individual Service Plans (ISP), and facilitate goal reviews and planning team meetings for direct services, including necessary travel for assigned caseload.

Manage a caseload of 100+ families from referral to service initiation, ensuring a streamlined process.

Utilize person-centered planning techniques to establish tailored goals for program participants.

Explain service models, staffing options, initiation processes, and ongoing benefits to participants and their support networks.

Provide guidance to Social Recreation Direct Support Providers using person-centered methods for problem-solving and goal achievement.

Identify high-risk situations, consulting with the Program Director to develop risk mitigation plans aligned with person-centered approaches.

Drive quality outcomes through regular progress reporting and timely service plan adjustments.

Coordinate with Regional Center staff and 24 Hour Home Care departments to ensure seamless service delivery.

Address consumer inquiries and concerns via various communication channels promptly and professionally.

Assist other Disability Services departments as needed, including provider enrollment and customer service.

Maintain and update electronic records daily for employees and program participants.

Support Community Partnerships and Program Director in fostering relationships with referral sources.

Serve as a subject matter expert for Social Recreation authorizations to support billing operations.

Conduct performance evaluations for direct support providers, offering feedback and recognition.

Uphold key performance metrics such as ISP completion rates and compliance with program admissions.
