The Integrated Care Center Clinic is a federally qualified health centers that provide primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We provide care to those most in need including those who are suffering from homelessness, mental illness, substance use, mental illness, and chronic medical conditions. We provide team based care in an integrated care setting. The Mobile and Street Medicine program delivers comprehensive, high-quality medical care and social services to individuals experiencing homelessness in San Francisco. Rooted in the mission to "go to the people", the program offers services directly to patients on the street, in encampments, or via a fully equipped mobile medical van. The program operates as an extension of our Integrated Care Center (ICC), expanding the reach of our services beyond the clinic walls. This new and rapidly growing initiative is committed to providing low-barrier, trauma-informed, and person-centered care in the environments where people live. Our multidisciplinary team prioritizes trust, continuity, and access while delivering compassionate, dignified, and high-quality services to some of the city’s most underserved populations.
The Clinical Social Worker provides behavioral health therapy and integrated case management to individuals experiencing unsheltered homelessness and complex medical, psychiatric, and social needs. This role combines field-based case management with direct clinical mental health services and plays a key role in strengthening and expanding the Mobile & Street Medicine program’s behavioral health capacity. This position is highly collaborative, flexible, and hands-on. The ideal candidate thrives in dynamic field environments, enjoys wearing many hats, and brings creativity, leadership potential, and a commitment to building new systems of care alongside a multidisciplinary team. This is a full-time, in-person, field-based position. This position may be filled by an ASW practicing under clinical supervision, which will be provided by an LCSW employed by HR360, or by an independently licensed LCSW.
KEY RESPONSIBILITIES
Behavioral Health and Clinical Services
◦ Provide mental health assessments, diagnosis, and ongoing psychotherapy to individuals with serious mental illness, trauma histories, substance use disorders, and co-occurring conditions.
◦ Effectively de-escalates clients experiencing psychiatric or emotional crises and facilitates appropriate interventions to ensure safety and continuity of care.
◦ Utilize evidence-based, trauma-informed modalities appropriate for street-based care (e.g., harm reduction, motivational interviewing, CBT-informed approaches).
◦ Collaborate closely with psychiatry, addiction medicine, primary care providers, and nursing staff.
◦ Participate in interdisciplinary case conferences and behavioral health consultations.
◦ Engages collateral contacts and utilizes appropriate referral sources, including hospitals, inpatient units, community providers, and support systems, to support comprehensive client care.
◦ Contribute to the development of workflows, protocols, and program design to expand the team’s behavioral health capacity.
◦ Provide Enhanced Care Management (ECM) services in alignment with Medi Cal Managed Care Plan requirements, including comprehensive biopsychosocial assessments and individualized care planning.
◦ Provide intensive, non ECM case management services for individuals with complex medical, behavioral health, and social service needs who require frequent coordination and support but do not meet ECM eligibility criteria.
◦ Maintain an active caseload of individuals experiencing homelessness with complex medical, behavioral health, and social needs.
◦ Develop, implement, and regularly update individualized, client-centered care plans in collaboration with patients and the multidisciplinary Mobile & Street Medicine team.
