Guardian Angel Senior Services is looking to add a new Home Health Aide to join our Salem, Ma team. The Home Health Aide will assist residents in their homes with all daily activities. These activities include everyday living, housekeeping, bathing and dressing, transportation, cooking, shopping, toileting, medication, mobility, and continence care.
The ideal person for this position has the experience, compassion, and a solid understanding of all aspects of patient home health care.
Tasks:
The Home Health Aide assists in providing nursing care per the care plan as in the following tasks:
- Supervise and Assist in Activities of Daily Living (ADLs) to include but not limited to:
- Bathing
- Dressing
- Grooming
- Toileting
- Encourages client in self-help or participation in activities to maintain independence.
- Assists and encourages client to follow prescribed exercise program which may include range of motion (ROM) exercises, and walking.
- Provides and assists with skin care by turning and re-positioning client as prescribed; as well as providing care to pressure point areas (e.g. back rubs).
- Assists and guides client in transfer process from bed to chair, commode or other devices.
- Assists in elimination process by giving bedpan, urinal, or bringing client to the bathroom; changing undergarments or emptying catheter bags. Measures and records intake and output. Sanitize all DME’s as prescribed.
- Assists in giving client bath, whether complete, partial, tub, or shower. Observes and reports changes in skin or physical condition.
- Provides or assists in oral hygiene, shaving and hair care.
- Takes and records vital signs to include temperature, pulse, and respirations.
- Changes simple dressing (non-sterile dressing only) and reports changes in wound status.
- Shops for food and household supplies. Plans, prepares, and serves meals according to dietary instructions.
- Maintains a clean, safe environment for client by performing household services such as:
- Cleaning and changing bed linens
- Cleaning floors with mop and/or vacuuming
- Cleaning bathroom and kitchen
- Laundering and ironing client clothes
- Uses and maintains special equipment and supplies appropriately.
- Informs supervisor of any change in client’s condition or family situation.
- Participates in the development and implementation of established care plans; communicating changes with office staff.
- Accompanies client outside the home when permitted
- Documents in Communication Book; recording and reporting any changes in symptoms, reactions or responses by client to Supervisor in an accurate and timely manner.
- Reminds client to take prescribed medications that are ordinarily self-administered. In certain approved situations: administer or apply medications.
- Tests urine for sugar and acetone and records and reports results.
- Performs additional tasks that may be assigned by the primary nurse or therapist, depending on the specific situation, on the Home Health Aide’s experience and competence. (Example: Routine uncomplicated colostomy care and decubitus care.)
- Provides or assists with postmortem care.
- Performs only those duties listed on care plan except when instructed otherwise by clinical supervisor.
