What You’ll Do:
• Conduct in-home client assessments
• Assist in developing personalized care plans
• Perform supervisory visits to ensure quality care
• Support caregivers and collaborate with families

Qualifications:
• Willing to travel (we will pay mileage)
• Active LPN license in Georgia
• Home care or case management experience preferred
• Strong communication & organizational skills
• Valid Driver’s License
• Social Security Card
• Recent negative TB test
• Current CPR certification
