This position is part of a growing, team-based home health program that provides care in patients’ homes, serving a diverse population including post-acute, chronically ill, prenatal, postpartum, and high-risk pregnancy patients. Team members can expect a mobile work environment with regular travel between patient residences. The role follows a structured weekday schedule with some flexibility based on patient needs and works closely with a multidisciplinary team to deliver coordinated, compassionate care. Currently Monday-Friday 8a-5p with expanded hours in the future.

One Health at Home:
One Health at Home is a program designed to deliver high-quality, patient-centered care in the home setting. It supports the work of our Advanced Primary Care clinic by extending care to patients with complex needs, including post-acute, chronic, prenatal, and postpartum care through in-person visits. The care team provides services such as chronic disease management, maternal health support, IV therapy, and coordination of labs and imaging in the home.
RN role for the One Health at Home program will be primarily site-based with some travel for home visits, as needed. Responsibilities include patient assessments, chronic condition monitoring and care coordination. The RN will work closely with a multidisciplinary team to support patients in their homes and communities.
We’re Looking For:
Completion of an accredited or approved program in nursing
Registered Nurse license issued by the state in which the teammate practices
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association
Experience in home health, chronic disease management, Primary Care, OBGYN or community-based care preferred. Previous supervisory experience preferred.
BSN preferred.
