RN Case Manager Responsibilities
- Oversee and manage designated Nursing Home household with a person-center focus
- Guide household staff in meeting the physical and psycho-social, person-centered needs of residents
- Responsible for medication/treatment administration as needed
- Admit, discharge and transfer residents to/from the household
- Complete the Resident Assessment Instrument (RAI) including MDS assessments, CAA’s, Triggers and Care Plans for assigned household members
- Perform skills requiring RN licensure such as IV therapy and blood draws
- Process and transcribe physicians orders according to policy and procedure
- Maintain up-to-date knowledge of emergency procedures and respond to emergencies

Qualifications
RN Case Manager Qualifications
- The RNCM must have a current license to practice in the state of Minnesota
- Must have a current Basic Life Support (CPR and AED) certification
- A minimum of two years nursing experience (long term care preferred)
- The RNCM must demonstrate leadership abilities to develop and motivate people; possess good critical thinking and problem-solving skills; must prioritize responsibilities; be flexible and able to work under stressful situations
- Possess good verbal and written communication skills (English); be self-directed and work autonomously and as a team member
- Management skills necessary to plan for and oversee the utilization of resources and the design of work processes
