RN Case Manager Responsibilities

  • Oversee and manage designated Nursing Home household with a person-center focus
  • Guide household staff in meeting the physical and psycho-social, person-centered needs of residents
  • Responsible for medication/treatment administration as needed
  • Admit, discharge and transfer residents to/from the household
  • Complete the Resident Assessment Instrument (RAI) including MDS assessments, CAA’s, Triggers and Care Plans for assigned household members
  • Perform skills requiring RN licensure such as IV therapy and blood draws
  • Process and transcribe physicians orders according to policy and procedure
  • Maintain up-to-date knowledge of emergency procedures and respond to emergencies

Qualifications

RN Case Manager Qualifications

  • The RNCM must have a current license to practice in the state of Minnesota
  • Must have a current Basic Life Support (CPR and AED) certification
  • A minimum of two years nursing experience (long term care preferred)
  • The RNCM must demonstrate leadership abilities to develop and motivate people; possess good critical thinking and problem-solving skills; must prioritize responsibilities; be flexible and able to work under stressful situations
  • Possess good verbal and written communication skills (English); be self-directed and work autonomously and as a team member
  • Management skills necessary to plan for and oversee the utilization of resources and the design of work processes
