Summary: Responsible for supporting the workflow of the Director of Quality and Risk Management with a focus on patient care gap closures and preventative health care. Assists with phone calls, mailers, scheduling and confirming appointments, and follow up appointments based on patient test results. Is knowledgeable in EMR documentation and providing timely communication for Provider follow up. Actively supports BCHC in its effort to maintain designation as a Federally Qualified Health Center (FQHC) and as a Patient Centered Medical Home (PCMH). Participates actively in BCHC’s mission to provide exceptional patient- centered preventive healthcare for all residents of Berks County, regardless of patient status.Please note this position is posted on behalf of our partner practices.
