Licensed Practical Nurse

Part-Time Evenings 3pm-11pm

Pay Range: $29.00 - $35.20 / Hourly (Pay information will vary depending upon relevant experience for the position.)  

**Generous shift differential offered for evenings, nights and weekends. 


The Licensed Practical Nurse is responsible for resident care activities under the direction of a Professional Registered Nurse.


ESSENTIAL JOB FUNCTIONS

  • Assure supervision of residents at all times.
  • Provide encouragement of residents in performing activities of daily living and assign appropriate staff to meet the care needs of the residents as needed.
  • Monitor physical, emotional, social and cognitive functioning levels of residents.
  • Work with team to arrange transfer to another care level if needed.
  • Keep the physician and families updated on changes in the resident status and ensure the physician instructions regarding care are met.
  • Assist the resident care director r in reviewing all medical information.
  • Review all DOH 3122s to ensure accuracy.
  • Keep resident care director updated on medication changes when needed.
  • Assess each resident’s health and functional abilities.
  • Communicate with residents and families regarding health status.
  • Work with residents’ primary physicians to coordinate care needs and medical orders.
  • Maintain all resident nursing records as required.
  • Maintain resident confidentiality.
  • Assign and direct all resident care in absence of resident care coordinator.
  • Develop schedule for all nursing staff to ensure staffing coverage at all times.
  • Supervise resident care assistants to ensure accurate medication administration & care needs are met for residents. 
  • Assume responsibility for proper storage of all medications not kept by the residents.
  • Maintain Optimus eMAR & task list per MD orders, DOH 3122, FA & ISP 
  • Phone in refills for residents on med-assist program and notify resident’s family if they pick up refills.
  • Monitor Optimus exception reports to ensure proper medication administration.
  • Assist resident care director with discipline of RCAs as needed.
  • Complete monthly medication review with residents who self-manage medications.
  • Provide resident teaching re: medication and medical equipment as needed.
  • Communicate all pertinent information to other staff.
  • May be assigned on-call duty on a rotational basis to handle resident and program emergencies as needed.
  • Conduct in-service training for resident care assistants as needed & offer continuous Optimus training & support to RCAs.
  • Assist resident care coordinator with in-service training as needed.
  • Ensure that resident care assistants follow policies and procedures.
  • Participate in quality assurance process.
  • Conduct audits regarding nursing program as needed.


 

Requirements

QUALIFICATIONS: 

  • Current NY state Practical Nurse Licensure.
  • Minimum one year clinical experience, preferably in gerontology.
