Responsibilities: Under the supervision of the Outreach Case Management Coordinator, serves as a nurse, performing a variety of direct and indirect duties. Leads the Healthcare Integration Initiative and the Hypertension Pathway Initiative. Responsible for consultation with providers and case documentation. Participates in interdisciplinary determination of care and treatment ensuring the coordination of medical, mental health, and substance abuse services to consumers served by the program. May render general nursing care and provide medical support in accordance with approved nursing practices. Additional responsibilities may include, but are not limited to: providing information, performing triaging for admissions screening, serving as agency liaison, performing crisis intervention activities, providing case management, providing patient education, and providing above services on a mobile basis as indicated. Responsible for carrying out all activities of the program/sub-unit in such a manner that fulfills CMHA-CEI’s mission, policies and procedures.
Requirements: Possession of an Associate’s degree in Nursing required. Possession of a State of Michigan license as a Registered Nurse required. One year of experience (professional or paraprofessional) in treating or working with a person who has a mental illness. Case Management duties may only be assigned when the employee has or attains one year of experience. Possession of First Aid and CPR Certification required. Ability to communicate effectively both verbally and in writing is required. Possession of a valid Michigan driver’s license and access to reliable means of transportation for job related use is required.
Conditional Employment Requirements: Employment offers for this position will be contingent upon the results of background verification and credentialing procedure consisting of: Michigan State Police clearance check, verification of a professional license, Medicaid/Medicare verification, verification of an acceptable driving record, educational background check, and verification of an acceptable work history. An individual may be excluded from consideration based upon their driving record. Must pass a pre-employment 5-panel drug screen.

Salary/Hours: $35.09 - $41 hourly. 40 hours per week.
Location: Adult Mental Health Services/Outreach Case Management, Lansing, MI.
