THE ROLE
Position purpose is to provide leadership and overall management of ElderPlace program operations, a program for All-inclusive Care for the Elderly. This Director position will have responsibility for 4 of our Health and Social Centers, Support Services including participant transportation, food services and medical equipment and supplies.
Leadership emphasis is on delivery of high quality care and services to participants, management of a complex budget in a managed care environment and the vision to create a work environment that promotes the employer of choice principles of Providence Health & Services. Accountable for delivery of the services consistent with OR Region strategic imperatives such as the Triple Aim.
MAJOR CHALLENGES:
- Effectively manage across multiple sites a complex, multi-dimensional program within allocated resources.
- Continual improvement of the inter-disciplinary teams functioning for improved service and utilization management.
- Successful recruitment of all health related disciplines to effectively staff the care teams.
- Create a dynamic vision for ElderPlace Operations that engages and stimulates all staff.
Patient Population Served: Adult, Geriatric
Consequently, the employee must be competent in the interpretation of a patient's self-report or behavior, and this information must be interpreted with an understanding of the cognitive, physical, emotional/psychosocial and chronological maturation process. The treatment of patients should be individualized and should reflect an understanding of the developmental needs and range of treatment needs for each patient.
ESSENTIAL FUNCTIONS
- Provides leadership for the successful achievement of ElderPlace Operations ensuring that quality integrated services are provided within the scope and intent of the PACE model.
- Manages personnel within the context of PHS philosophy and policies to ensure a safe and challenging work environment. Provides leadership in salary and benefit review, recruitment and retention of quality employees including selection, development, motivation and evaluation.
- Participates in development of, and delivers on all, quality, service delivery, utilization and financial targets established for ElderPlace operational departments reporting to this position.
- Provides effective leadership to accomplish the purposes of Service Excellence including employee engagement and development programs.
- Establishes liaisons with local and state industry representatives to further the understanding of PACE.
- Provides leadership in developing operations goals and objectives that support the ElderPlace vision and recommending long-range plans for operational objectives. Participates in ElderPlace strategic planning.
- Collaborates with Director of Finance in developing an appropriate budget for assigned departments to ensure quality provision of services within resources available, ensuring a net revenue that promotes viability of the program. Establishes effective budget management objectives that direct the utilization of resources and meets the commitments of the annual budget. Accountable for financial performance for departments reporting to this position.
- Participates in activities aimed at program growth including marketing strategies to gain improved market share, including establishment of a successful record of customer service with referral sources.
- Collaborates with quality leadership to ensure ElderPlace operations has a key role in developing and achieving quality initiatives that continually improve the care to participants and contribute to the efficient operations of the program.
- Collaborates with clinical leadership in development, implementation and monitoring for the best geriatric clinical practices available for the frail elderly population.
- Establishes strong working relationships with other Directors within PEPP to achieve overall success of the ministry.
- Works closely with community partners (e.g. Volunteers of America) to further PEPP's ability to meet community need and provide services for frail elders.
- Responsible for oversight of contracted entities providing services to participants assigned to Director's health and social centers.
- Ensures compliance and high quality implementation of Ministry-wide program initiatives, such as PACE at Home and Advanced Care Planning.
- Negotiates with vendors on behalf of PEPP as requested.
- Stays abreast of all legislative and regulatory issues affecting PACE and related programs.
QUALIFICATIONS
- Bachelor's Degree in a related field; or equivalent educ/experience
- Master's Degree (preferred)
