About the role

Description

The Therapeutic Foster Care Supervisor will be working with the TFC team and provide supervision and support of the TFC parent to help support clinical interventions that will bring about change in the consumer receiving TFC services.

Requirements

Master Level Applicants:

Must have degree in Social Work or related field

Minimum of two years paid experience with the population served

Sound knowledge of the service provision.

Qualified Mental Professional status required

Bachelor Level Applicants:

Must have degree in Social Work or related field

Minimum of one-year paid experience with the population served

Sound knowledge of the service provision.

Qualified Mental Professional status required

Experience Preferred:

Experience licensing and training foster families

Experience working in a team environment with MST or Intensive In-home Services

Experience in program management and development

Responsibilities :

· Supervise and support TFC families and provide guidance to the TFC parents while staying in compliance with MCO, CARF and regulatory and licensing requirements as determined by the State

· Manage admit/discharge aspects of TFC in the CCW

· Audit TFC charts monthly to ensure consents, assessments and PCP/CCP are current and present for all FC consumers

· Complete monthly Together Facing The Challenge staff coaching form

· Facilitate 90 day and annual evaluations for all TFC staff

· Facilitate new hire training/orientation for TFC service

· Monitor and consult with staff in regard to time off requests

· Review staff documentation on a weekly/monthly basis to ensure documentation meets requirements

· Provide monthly on call support (rotation), consultation and collaboration with staff on identifying problem areas, developing plans for addressing those problems in order to minimize the risk of placement disruption.

· Maintain up-to-date documentation as related to TFC home licensure, contractual agreements, and all agency and regulatory guidelines. Completes annual evaluations on IAFT parents.

· Responsible for creating and collecting initial clinical documents(PCP, CCP, CCA, Justification).

· Responsible for completing initial NCTOPPS, CCW, Ontarget
