Overview
Valley Senior Living is seeking a dedicated Licensed Social Worker to join our care team. In this role, you will provide medically related social services that support residents in our skilled nursing facilities. Your work will focus on helping residents achieve and maintain their highest level of physical, emotional, and psychosocial well-being.

As an integral care partner, you will play a key role in honoring each resident’s individual needs, preferences, and life experiences. Guided by our mission, vision, and values, you will contribute to a culture of person-centered care where every resident is respected, supported, and empowered.

Qualifications

  • Education: Bachelor’s degree in Social Work required.

  • Experience (Preferred):

    • One or more years of professional social work experience.

    • Experience in long-term care or with the geriatric population.

    • Membership in Long Term Care Social Workers of North Dakota.

  • Licensure: Must hold, or be in the process of securing, a current North Dakota Social Work license.

Key Responsibilities

    • Support residents and families through transitions, admissions, discharges, and room changes.

    • Provide therapeutic interventions to help residents adjust and thrive within our community.

    • Act as a resource and advocate for residents, ensuring their rights are respected and their voices are heard.

    • Collaborate with interdisciplinary care teams to develop and implement care plans.

    • Coordinate with community resources, health providers, and agencies to meet resident needs.

    • Document assessments, interventions, and care plans in accordance with federal, state, and facility regulations.

    • Contribute to a positive community culture by supporting resident councils, care conferences, and facility committees.

    • Maintain compliance with HIPAA, corporate compliance standards, and safety procedures.
