Jail Transition Therapist - Full Time - Southwest, Colorado

Position Summary: The role of the Therapist is to participate as a member of the Jail Transitions program team to intensively support patients by initiating services while incarcerated and to continue services into the community. The services shall include designing and implementing the individual care plans, providing therapeutic interventions, referral, linkage, and advocacy for program patients to prevent recidivism.  The therapist will work with other community programs to ensure full integration into the community for the patient. Services may be provided in La Plata and Montezuma counties. When not working with jail population, provides general outpatient treatment. 

Participates as a member of the program team to design and implement the individual care plans for program clients in accordance with policies, rules, regulations, and directives which apply.  Utilizes a variety of treatment approaches and case coordination to work with a diverse client population. 

Our ideal candidate for our Therapist position will have their master’s in one of the recognized mental health professions, be Colorado License Eligible/Licensed LPC, LMFT, LCSW or PsyD in the State of Colorado and have a minimum of (2) two years’ experience in mental health field.  Experience in Substance Use Treatment preferred.

 Salary:

• Therapist-Unlicensed: Starts at $21.00 - $26.52   Non-Exempt
• Therapist-Licensed: Starts at $58,240.00 - $ 72,508.80 Exempt
