JOB SUMMARY
Provides physical therapy treatment to patients who have impairments, functional limitations, disabilities, or changes in physical function and health status resulting from injury, disease or other causes under the direction of a Physical Therapist. Actively collaborates with the health care/educational team to meet patient care, outcomes and system improvement objectives.

Physical Therapist Assistant (PTA) - Home Health

Service Areas: Rutherford, Sumner, and East Davidson Counties

Schedule: Usually Monday–Friday, 8:00 am – 4:30 pm, but we also offer 4- and 5-day workweek options.

Vanderbilt Health is seeking a compassionate and skilled Physical Therapy Assistant (PTA) to join our Home Health team. In this role, you will deliver high-quality, evidence-based physical therapy treatments to patients with physical impairments, functional limitations, or disabilities resulting from injuries, illnesses, or chronic conditions. You will work under the supervision of a licensed Physical Therapist and collaborate with a multidisciplinary healthcare team to support patient recovery and independence in the home setting.

How You’ll Make a Difference:

  • Deliver individualized physical therapy interventions based on the treatment plan developed by the supervising Physical Therapist.

  • Promote patient safety and functional independence in the home, community, and vocational environments.

  • Educate patients and caregivers on therapeutic exercises, mobility strategies, and home safety.

  • Accurately document patient progress and treatment outcomes in compliance with VUMC and regulatory standards.

  • Collaborate with nurses, physicians, and other allied health professionals to ensure coordinated care.

  • Travel to patient homes within Rutherford, Sumner, and East Davidson Counties.

Required Qualifications

  • Active Physical Therapist Assistant (PTA) license in the state of Tennessee.

  • Minimum 1 year of experience as a Physical Therapist Assistant (PTA)

  • Valid driver’s license and reliable transportation.

Preferred Skills & Competencies

  • Proficiency in electronic medical records (EMR) and documentation systems.

  • Strong understanding of rehabilitation techniques, functional mobility, and evidence-based therapy.

  • Excellent communication and interpersonal skills.

  • Ability to work independently and manage a flexible caseload.

KEY RESPONSIBILITIES
• Applies appropriate, evidence-based interventions to ensure effective outcomes based on the treatment plan of the primary clinician.
• Provides education and employs strategies to promote successful return to home, community, and school/vocational environments.
