Care Coordinator, Inpatient Behavioral Health

The purpose of this position is to provide discharge planning services, initial assessment and support to patients hospitalized on the inpatient behavioral health services. The inpatient care coordinator reports to the Inpatient Medical Director for Behavioral Health. They work closely and in collaboration with other inpatient staff including psychiatrists, mid-level providers, therapists, nurses, program specialists and aids, and clerical support staff.

Qualifications 

• Bachelor's degree in human services field from an approved educational program
• Minimum one year experience working in the mental health field
• Ability to work cooperatively with a multi-disciplinary team and to partner with other social workers and therapists
• Competence with computers and software (Outlook, Word, and Excel)
• Excellent written and verbal communication skills
