Schedule Details: M-F 8:00am-4:30pm; No scheduled weekends; no holidays 

 

What You Will Do:

Promote positive patient outcomes and satisfaction by optimizing support of oncology providers' decision-making with evidence-based, safe and cost-effective pharmaceutical based therapies. In addition, provide direct patient education for oral chemotherapy regimens and follow-up monitoring for compliance and toxicity management.

 

Responsibilities:

  • Provide direct oncology provider consultation for evidence-based, cost-effective treatment options.
  • Review of new oral chemotherapy treatment plans for drug-drug, drug-lab, drug-food interactions and appropriateness of therapy
  • Counsel oncology patients on new oral chemotherapy (and IV therapy as needed) to minimize toxicity and enhance patient understanding of compliance.
  • Assist/recommend/manage adverse drug events related to oral chemotherapy (and others as applicable).
  • Documentation of patient interactions in EMR for oncology team transparency.
  • Documentation in EMR for other actions taken.
  • Assist Medication Access Specialists with oral chemotherapy insurance prior-authorizations.
  • Assist with Medication Access Specialists with patient co-pay assistance programs or other programs to defray financial toxicity to patients.
  • Coordinate with Medication Access Specialists oral chemotherapy delivery timelines for patient expectation setting.
  • Assist in development of pharmacist-driven delegation protocols such as symptom management protocols
  • Utilizes Collaborative Practice Agreement (CPA) to order labs, supportive care treatments. 
  • Assist/Lead program development programs such as Next Generation Sequencing (NGS), CAR-T, BiTE, etc.
  • Lead practice-based interdisciplinary committees like Oncology P+T and others as assigned.
  • Prepares and presents new medications for formulary review.
  • Monitors outcomes of clinical oncology pharmacy services to ensure services are cost-effective, data driven and value added
  • Coordinates policies and procedures for clinical activities
  • Oversee Beacon protocol development and coordinate rare treatment situations
  • Assist with operations in Oncology Infusion Pharmacy as needed.
  • Precept pharmacy students, interns, and/or PGY-1 or PGY-2 Oncology residents as available.
  • Competency completion/development as assigned.
  • Assist in writing Letters of Medical Necessity for providers

 

Requirements:

  • Doctor of Pharmacy (PharmD)
  • State of Wisconsin Pharmacist license (or license eligible)
  • Basic Life Support Certification (BLS) 
  • Specialized expertise in Oncology Pharmacy
  • Board Certified Oncology Pharmacist (BCOP) preferred
  • Valid Driver's License
  • Strong oral and written communication skills; proficiency in Microsoft Office 
  • Ability to communicate in Spanish preferred
