Are you passionate about making a real difference in the lives of others? Trinity Living Center is seeking a compassionate and skilled Health Care Coordinator to oversee medical and health services for our valued participants.
Position Details
- Job Title: Health Care Coordinator
- Location: Trinity Living Center, Salisbury, NC
- Schedule: 30 hours per week, Monday through Friday (9:00 AM – 4:00 PM or 10:00 AM – 5:00 PM). No weekends or holidays!
- Pay Rate: $25 – $30 per hour

Key Responsibilities
- Oversee and supervise medical and health services for participants
- Monitor health and safety at all times
- Administer and manage medications according to state regulations
- Coordinate with health care professionals, family, and caregivers
- Complete health assessments for new and existing participants
- Develop and monitor individual service plans
- Provide first aid, maintain emergency files, and handle incident reporting
- Train and educate staff on health policies, emergencies, and procedures
- Assist participants with daily living activities as needed
- Monitor dietary needs and support infection control procedures
- Maintain positive relationships with participants, families, and staff
- Keep inventory of medical supplies and ensure safety standards are met
- Reflect LSC’s mission, vision, and customer service standards every day
Qualifications
- Current R.N. license to practice in North Carolina (Willing to look at LPN applicants as well)
- At least 2 years of experience, including supervisory skills
- Strong managerial, communication, and administrative abilities
- Proficiency in Microsoft Office
- Knowledge of aging, related illnesses, and rehabilitative care
- CPR and first aid certified
Preferred Background
- Experience in adult day services settings
- Experience working with older adults, disabled adults, or adults with dementia
Physical Requirements
- Ability to lift up to 35 pounds and assist with repositioning or transferring individuals as needed
- Ability to bend, stoop, and work at floor level.
