Overview

Community Health Partners (CHP) is seeking a full‑time Nurse Practitioner (NP) or Physician Assistant (PA) to join our established outpatient Medical Oncology practice in Fresno, CA. This is an exceptional opportunity to work alongside a collaborative team of Medical Oncologists, Hematologists, and experienced Advanced Practice Providers in a comprehensive cancer center dedicated to delivering high‑quality, patient‑centered oncology care.

Responsibilities

Provides comprehensive oncology assessment, evaluation, and direct patient care in accordance with established protocols.

Manages patients collaboratively with supervising physicians, including participation in daily rounds, treatment planning, and follow‑up care.

Orders, interprets, and evaluates diagnostic tests such as CBC, CMP, tumor markers, imaging studies, and pathology reports.

Furnishes medications—including chemotherapy‑related supportive agents—based on standardized procedures.

Educates patients and families on diagnosis, treatment options, chemotherapy/immunotherapy regimens, side‑effect management, and survivorship planning.

Coordinates referrals to radiation oncology, surgical oncology, and other subspecialties as needed.

Obtains prior authorizations for oncology medications, imaging, and procedures.

Documents patient encounters thoroughly and accurately in the electronic medical record.
