Registered Nurse PRN Inpatient Unit

Principle Duties and Responsibilities:

• Completes an initial assessment of patient and family to determine hospice needs.
• Provides a complete physical assessment and history of current and previous illness (es).
• Provides professional nursing care by utilizing all elements of nursing process.
• Assesses and evaluates patients status by: A)Creating and initiating Plan of Care, B) Regularly re-evaluating patient/family/caregiver needs, C) Revising the Plan of Care as necessary.
• Develops a care plan which establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Contributes as a team member to identify needs for patient plan of care.
• Assists in conducting clinical observation of CNAs and LPN’s in preparing written evaluations.
• Follows HOP personnel policies and procedures.
• Works in cooperation with the family/caregiver and Hospice Interdisciplinary Team Members to meet the emotional needs of the patient and family.
• Participates in the Interdisciplinary Group meeting as requested/assigned. 

Requirements

Qualifications:

• Graduate of an accredited nursing school
• Licensed as a Registered Nurse (RN) in the Commonwealth of Virginia
• Ability to manage all aspects of terminal care, health team service coordination, and documentation.
• CHPN Preferred.
• Basic knowledge of Medicare/Medicaid reimbursement regulations.
• Emotionally mature and caring individual willing to give support and assistance to terminally ill and their families.
• Excellent interpersonal and communication skills.
• Must demonstrate basic computer skills.
