If you are looking for a role where you can provide one-on-one independent PT management, services, recovery, and empowerment, leaving a positive impact on the lives of your patients, come join our wonderful team!</p> <p>In this role, you’ll help patients regain mobility and independence in the comfort of their homes — <strong>working with a supportive interdisciplinary team that values collaboration, flexibility, and clinical excellence. </strong>The Physical Therapist provides evaluation and therapeutic services in the home under a plan of treatment. Care is directed at rehabilitation to maximum potential, as well as promotion of independence. Teaching patients rehabilitative and therapeutic procedures while in their home setting is a powerful addition to successfully meeting recovery plans.</p> <p>We take pride in serving our community with compassion and respect, focusing on the <strong>physical, emotional, and spiritual well-being</strong> of each patient and their family.</p> <p><strong>What You’ll Do</strong></p> <p><strong>Patient Evaluation & Care Planning</strong></p> <ul> <li>Conducts comprehensive <strong>initial and ongoing assessments</strong> of patient mobility, strength, balance, coordination, pain, and functional status.</li> <li>Evaluates home environments for safety and recommends adaptive equipment or modifications as needed.</li> <li>Develops and implements individualized <strong>Plans of Care (POCs)</strong> based on patient needs and physician orders.</li> <li>Establishes measurable goals and expected outcomes to restore and/or improve function.</li> </ul> <p><strong>Treatment Implementation&l
