About the role

The Role:

  • The CNC supports the service by completing the initial triage, completion of patient assessments and managing follow reviews.
  • Provides supervision, guidance and nursing support to the multidisciplinary team  .
  • Develops, implements, and evaluates strategies to ensure effective and efficient delivery of care and drive improvements as needed.
  • Ability to work as an independent practitioner and a key team member
  • Experience in acute nursing is strongly regarded.
  • Excellent communication and relationship building skills

The Team:

The Rapid Access Inreach Service (RAIS) supports the care of individuals in the community, it is a recognised hospital avoidance initiative, preventing unnecessary emergency department presentations and hospital admissions. The team is multidisciplinary with Medical Physicians, Nursing, Administration and Allied Health team members all who work in collaboration with General Practitioners and District Health Services.

Details of appointment:

Fixed term part time, day worker position, working up to 16 hours per fortnight, as soon as we can onboard you until 31 March 2026.

Note: Position is based at the Launceston General Hospital with a requirement to travel to different district locations across the Northern regions of Tasmania

*notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $111,506 to $118,336 pro rata per annum. Our Employer 11.5% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2023.

Eligibility:

Successful applicants will be required to meet the essential criteria.

  • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse
