About the role

The LPN is responsible for administration of all medications and treatments during their shift, according to the established medical and nursing care plan. Works under the direction of the Head/Charge Nurse to provide optimum nursing care to all residents on the unit. 

  • Continuously evaluates behavior, symptoms, reactions, and progress of residents and discusses findings with Nurse Manager/Charge Nurse. 
  • Administers and documents all medications according to established policies and procedures, physician's orders, and Department of Health regulations. 
  • Performs all treatments according to physician’s specific order and established policies and procedures. 
  • Discusses the need to discontinue or change a medication of treatment with the Nurse Manager/ Charge Nurse. 
  • Orders prescribed medications, notifies pharmacy of discontinued medications: and ensures accuracy of medications and dosages when received from pharmacy. 
  • Coordinates the planning, implementation, and evaluation of resident care services on a 24-hour basis.
  • Utilizes knowledge of current trends and practice standards of nursing including geriatric, adult resident, and Assisted Living resident concepts to coordinate staff responsibility to provide quality care.
  • Monitors staff compliance with facility policy and procedures, as well as state and federal regulations.
  • Audits records to ascertain compliance with facility standards.
  • Directs and supervises PCA, Home Health Aide, CNAs, and other team members as necessary.

 

Qualifications: Current NY State Licensed Practical Nurse & BLS  

 

Education: Accredited LPN School
