Job Summary:

The Community Health Worker (CHW) will serve as a vital link between the PACE participants, healthcare teams, and the community. The CHW will support participants by assisting with care coordination, providing health education, addressing social determinants of health, and promoting wellness. This role involves advocating for participants, helping them navigate healthcare and social services, and fostering a trusting relationship to enhance their overall quality of life.

Education, Licenses, Certifications and Experience:

• High school diploma or equivalent.
• Certification or training as a Community Health Worker is preferred.
• Valid driver’s license and reliable transportation.
