Part-Time Licensed Practical Nurse/CMA - GSPN Washington Primary Care

1. Participates and assists in data collection in a systematic and organized manner to provide high quality and safe outcomes.
2. Contributes collected data to the development of a plan of care in collaboration with other healthcare professionals that contributes to high quality and safe outcomes.
3. Implements a plan of care with measurable criteria consistent with world class performance levels.
4. Collaborates with other team members to evaluate the plan of care with the patient and family/support system to maintain world class levels of patient satisfaction and outcomes.
5. Contributes to a high performance work environment by seeking educational resources that enhance and maintain current knowledge and skills.

Secondary Job Duties That May be Reassigned:

1. Performs other duties as assigned or requested

Job Specifications:

• Education: Graduate from an accredited school of practical nursing
• Licensure: Practical Nurse license from the Indiana State Board of Nursing. Some area may require a current and valid driver’s license.
• Experience: Graduate of LPN program
