Role Description
The Sr. Associate, Population Health supports high-impact care model initiatives aimed at improving quality and cost-effectiveness across care management, patient engagement, outreach, documentation, and third-party medical costs.
The ideal candidate is a self-starter, enjoys solving challenging problems, uses data to drive insights, and excels in a fast-paced environment. This will be a high-visibility role requiring significant cross-team collaboration within Population Health and other corporate teams (e.g., Business Intelligence, Care Services, Clinical Quality, Managed Care Operations, etc.).
Core Responsibilities
Support 1+ high-impact strategic initiatives that evolve the Oak Street care model across Care Management and Complex Patient Care
Partner with clinical and operational field leaders to support ongoing initiatives related to Care Management and Complex Patient Care, including strategies to support our highest risk patients
Manage detail-oriented projects requiring partnering across internal cross-functional teams, product teams, and decision sciences
Identify and implement opportunities to improve efficiency of workflows and overall project development.
Partner with clinical and operational field leaders to support ongoing initiatives related to Care Model Initiatives and Complex Patient Care with touchpoints with senior leaders
Own the analytics of large and complex data sets on an ongoing basis, as well as ad hoc analyses for strategic and operational initiatives
Develop metrics and reports to monitor success and identify opportunities for improvement within Complex Patient Care
Other duties as assigned.
What are we looking for?
3 - 5 years of prior relevant experience; 2+ years of consulting or other applicable experience in healthcare / population health preferred
Analytical and data savvy; highly proficient with Microsoft Excel / SQL
Strong interpersonal skills that support effective stakeholder engagement
Strong written communication skills; highly proficient with Microsoft PowerPoint
Ability to develop business cases, ROI analysis, and financial analysis
Passion for excellence, willing to go the extra mile to achieve objectives and ensure consistently high quality work product
Quick learner comfortable with a high level of ambiguity
Ability to succinctly summarize and synthesize large amounts of information
Strong accountability and discipline against timelines and deliverables
Collaborative working style, able to work cross functionally
