About Thrive: Thrive Lake County is a newer 185 bed high-end Skilled Nursing Facility that opened its doors in 2020, and we focus on patients of all ages. We have a large Transitional Care Unit, where most of our patients go home after (or before) 17 days, a dedicated Memory Care Unit and a long-term care unit. Requirements
Job Qualifications
Director of Social Services Qualifications: SNF/LTC
• At least one year experience required working with elderly population in acute, long term care, or community based setting, preferred.
• Compassionate and caring demeanor.
• Ability to build rapport with residents/families.
• Must be comfortable with one on one conversation, routine visits with residents, and facilitating difficult conversations with families.
• Demonstrate progressive leadership and initiative.
• Excellent written and verbal communication skills
