Registered Nurse Case Manager - Behavioral Health

Job Description

 

Spectrum Healthcare Resources has an opportunity for a civilian contractor for a Full time Registered Nurse – Behavioral Health/Case Manager  at the Fairfax Ambulatory Care Clinic in Fairfax, VA.

 

This contract position offers:

• Monday-Friday daytime - 40 hours (7:30am-4pm or 8am-4:30pm)
• Outpatient Setting
• Opportunity to Care for our Active Duty, Retiree's and their families
• 11 paid federal holidays (no weekend or on-call responsibilities) 

Essential Functions:

• Triage patients, eliciting needed information to determine the nature and urgency of Behavioral Health problems.
• Assist patients in finding appointments in the direct and purchased care BH system
• facilitate emergent care coordination and transfers for inpatient psychiatric hospitalization.
• Collaborates and plans for comprehensive health care with other healthcare team members.
• Works closely with the Behavioral Health provider team to monitor, track, and manage patients identified as “high risk”. 
• Functions as resource person and facilitate for patients, helps acquaint patient with available community resources
• Collaborates with Health Center Case Management staff to support on-going case management efforts for Behavioral Health patients with a requirement for Case Management tracking and assistance.

Job Requirements:

• Must be a U.S. Citizen.
• Shall have completed a Bachelor of Science Degree in Nursing (BSN) from an accredited college or university; and/or graduate of an accredited National League of Nursing (NLN) School.
• 2+ years of RN Behavioral Health experience preferred within last 5 years,
• Working knowledge of interdisciplinary treatment team process, behavioral health programming and daily nursing functions for outpatient behavioral health.
• Have knowledge of computer operations and proficiency in the use of basic word processing, data entry, and automated records.
