Role Description:

Oak Street Health is attracting and cultivating team members who embody our values and are passionate about the mission to rebuild healthcare as it should be.  Our focus includes Primary Care, Podiatry, and Behavioral Health.  Podiatry provides essential care for our patient population--especially for diabetics--and we have providers currently in the following clinic locations:

• Illinois:  Ashburn, Avalon Park, Bellwood, Bronzeville, Blue Island, Englewood, Chicago (Madison Street), Hermosa

• Indiana:  Fort Wayne, Gary, Glendale, Hammond, IRV, Speedway, UH

• Ohio:   Glendale, Lee Harvard, Westown

There are also plans for expansion throughout the country.    

We start simple--build meaningful relationships with patients and their families and then layer on resources to help our teams produce excellent care.  Our physicians practice with a full team behind them which is made up of the following:

• Registered Nurse

• Medical Assistant

• Clinical Informatics Specialist (i.e., a “scribe” to manage documentation)
