Job Summary:
Serve as the first point of contact for patients, their families and caregivers, and referral sources. Assist in contacting and screening patients for eligibility on the CHHA program. Follow patients from referral to discharge on the program to ensure a smooth and fulfilling experience by all patients.
Qualifications:
1. High school diploma
2. Good communication skills
3. 1 year of experience in health care intake
4. Understand the requirements for the CHHA
5. Understanding of communicating effectively with employees, patients and their families, medical and community affiliates to develop positive relationships.
6. Experience working in a computerized environment.
7. Absolute integrity and sufficient maturity to handle confidential aspects of work.
Physical Requirements:
1. Possess the sight/hearing senses or uses prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
2. Must the regulatory health status requirements of the NY State Department of Health
Work Requirements:
1. Work in the office environment with regular exposure to Administration, Management, office staff, vendor agency's personnel, patients, and their significant others.
2. Must be able to sit for prolong period, transport materials or equipment, lift, move or carry some items throughout the day.

Job Functions and responsibilities:
1. Receive, call, and screen referrals from referral sources such as: hospitals, nursing homes, rehab, MLTCs, and case managers.
2. Contact patients and educate them on the services we can provide and assist them with for their home care needs.
3. Screen clients for eligible for enrollment into program.
4. Follow up with social workers, case managers and referral sources to ensure a smooth transition from referral to patients.
5. Keep patients, their caregivers, and the referral sources updated throughout the entire admission-discharge process.
6. Enter patient demographics into patient care software system.
7. Schedule initial SOC appointments for Registered Nurses
8. Answer phone and email inquiries from prospective and current patients, referral sources, and case managers.
9. Address any patient complaints or concerns.
10. Contact HMO patients during the process of requesting authorization and update them of their status.
11. Administrative Functions:
a. Assemble patient charts
b. Assemble Intake packets as needed
c. File and organize active and discharged charts
d. Other administrative functions as needed
12. Other duties as assigned by Operations Manager
13. Participates in Performance Improvement projects.
14. Accepts and completes assignments as given within scheduled time frame.
15. Projects positive business-like demeanor. Promotes positive work environment.
16. Maintains confidentiality of patients' records
17. Accountable - maintains an acceptable record of attendance.
18. Complies with policies and guidelines established for meal periods, breaks, sick calls, requests for time off and dress code
