Job Description
Location: Ben Gordon Center - 12 Health Services Drive
Schedule:
Monday-Thursdays 7:45am-4:15pm
Friday 8:30am-5:00pm
The Behavioral Health team is seeking a Licensed Registered Nurse with either three years of nursing experience or one year of nursing experience and two years behavioral health experience
The RN Case Manager reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
Plans and provides patient care to achieve optimum patient outcomes. Employs a collaborative process for assessing, planning, implementing, coordinating, monitoring, and evaluating the options and services required to ensure that patients receive the appropriate level of care and services to meet their ongoing healthcare needs.
RESPONSIBILITIES
- Screen and evaluate requests for service.
- Intervene, stabilize, and manage acute crisis situations.
- Assess and evaluate client needs, strengths, goals and resources.
- Work with an interdisciplinary team in reviewing cases and planning treatment.
- Develop individual treatment plans.
- Screen for tardive dyskinesia for all clients prescribed phenothiazine medications.
- Updated diagnoses in consultation with Psychiatrist.
- Participate in a team approach to maintaining the caseload of medication monitoring clients of Rehabilitation Services Program (medication monitoring is defined as the monitoring, observation, and regulation of medication to a client as the primary services from KHS Behavioral Health Services).
- Lead medication groups and work with Psychiatrist(s) to coordinate services for clients in the medication groups.
- Implement treatment through various therapeutic modalities such as individual, family, group or conjoint therapy.
- Consult with other center staff regarding problem assessment and treatment planning.
- Consult with other service agencies around case-specific and programmatic issues.
- Serve as a liaison to other agencies as needed.
- Train other providers and non-providers in areas relating to service specialties.
- Meet with other nursing staff at least quarterly to review and discuss medical procedures and issues.
- Consult with program staff regarding medical/physical problems of patients.
- Assist program staff in planning treatment which will improve medical/physical problems of patients.
- When authorized by a physician, administer approved medication.
- Routinely meet with program staff in order to plan and discuss the performance and content of program medical aspects.
- Monitor all lab test results and convey information to Psychiatrist(s).
- Review physical health questionnaire and make recommendations as indicated.
- Assists with developing and implementing procedures.
- Report any problems to management and recommend solutions.
Qualifications
Required:
- Associate’s degree
- Licensed Registered Nurse
- Three years nursing experience or one year nursing experience and two years behavioral health experience
- CPR-Basic Life Support within sixty days of hire
- All NM, Ben Gordon Center (WR849) employees, at point of hire and annually, are required to have a valid State of Illinois driver’s license, current auto liability insurance, and an annual driving abstract
Preferred:
- Bachelors of Science in Nursing
- Two years Behavioral Health experience
