JOB SUMMARY
The Mobile Integrated Health (MIH) Specialist 3 works as part of a multi-disciplinary team to provide a variety of care in the community and home setting. The MIH Specialist plays a crucial role in improving patient health outcomes, preventing re-admissions to the Emergency Room, Rural Health Clinic, and Hospital level of care. The MIH Specialist serves as a communicator to other departmental team members when referral opportunities and care coordinator needs are recognized.
ESSENTIAL DUTIES AND RESPONSIBILITES
- Manages the referral dashboard in the electronic health record.
- Maintains knowledge of Ferry County Health nursing district policies.
- Communicates closely with external care partners to achieve timely social, medical, and behavioral health referrals as needed.
- Communicates clearly and professionally to deliver and document education about various health topics (such as diabetes prevention, hypertension, nutrition, and advanced care planning).
- Analyzes lab values and integrates them into patient assessments and clinical decisions.
- Provides a compassionate and calming environment for patients so any questions or concerns can be adequately addressed.
- Monitors the referral dashboard and accurately updates patient statuses.
- Inspects, cleans, and restocks the MIH truck to ensure that all the equipment and supplies are present, in good operating order, and ready for response.
- Drives the MIH truck safely as a representative of the district while also following all Washington State statutes and revised codes.
- Activates 911 emergency response when a patient at home is triaged and found to need emergency medical treatment.
- Administers patient care following established active protocols.
- Attends monthly Inter-Agency meetings with the Care Coordinator for collaboration and better understanding of local resources.
- Cooperatively works with community-based organizations to provide comprehensive patient centered community nursing care.
- Delivers MIH program updates to the Nursing department during staff meetings.
- Checks work cell phone voicemails daily and responds to patients timely.
- Reviews Meditech Workload messages daily and communicates clearly back to team members with status updates.
- Documents care provided, assessments, and phone communication events same day the in electronic medical record.
- Assists the Care Coordinator to conduct post-ER follow-up calls to identify patients in need of a potential MIH visit referral.
- Manages, coordinates, and participates in task force project assignments.
- Perform related duties as assigned by management.
PROFESSIONAL REQUIREMENTS
- Actively checks and responds to emails and secure qliq messages throughout the day.
- Completes all mandatory education within the given time-frame.
- Maintains regulatory requirements.
- Reports to work on time and as scheduled.
- Wears identification while on duty; uses computerized punch time system correctly.
- Represents the organization in a positive and professional manner in the community.
- Attends annual review and department in-services as scheduled.
- Attends at least 75% of Nursing Department staff meetings; reviews all monthly staff meeting minutes. (For internal candidates only)
- Actively participates in the department’s quality improvement (QI) activities.
- Complies with all organizational policies regarding ethical business practices and the employee handbook.
- Communicates the mission, vision and values of the organization as well as the focus of the department.
