Key Responsibilities

Clinical Documentation

  • Review and enter data in Remote Patient Monitoring (RPM) dashboards

  • Document all patient outreach encounters in Epic

  • Upload patient-specific information and updates to Epic

Patient Communication

  • Conduct outreach for both clinical and non-clinical patient needs

  • Perform patient triage using Health System–approved resources

  • Communicate patient outcomes and updates to managing providers to ensure continuity of care

Program Logistics

  • Coordinate delivery and return of monitoring equipment

  • Manage patient disenrollment from the RPM program

  • Maintain and update patient enrollment reports using Microsoft applications

Interdisciplinary Collaboration

  • Partner with interdisciplinary teams to identify and support opportunities for RPM program growth and improvement

Outcome Tracking & Quality Improvement

  • Use the RPM portal and Epic to monitor program success and patient outcomes

  • Identify trends and areas for process improvement

This position works under the general supervision of a physician, advanced practice provider, and/or nursing leadership to provide nursing services in care and treatment of patients.

EXPERIENCE AND EDUCATION

Required

  • Education
    Graduate of an approved/accredited nursing program with an Associate Degree in Nursing (ADN) 
     
  • Experience
    Specialty areas may have increased experience requirements as dictated by clinic or standard of care. 
     
  • Licenses and Certifications
    (RN) REGISTERED NURSE Licensure as Registered Nurse by Board of Nurse Examiners for State of Texas Upon Hire 
    BASIC LIFE SUPPORT Through course accredited by the American Heart Association (AHA) or American Red Cross (ARC) Upon Hire
