Pay Range: $28.30 - $38.91 Depending on Years of Experience


JOB PURPOSE: The Behavioral Health Care Manager - Collaborative Care is a core member of the collaborative care team, including the patient’s medical provider and psychiatric consultant, as well as the larger primary care team or medical team. The Behavioral Health Care Manager - Collaborative Care is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s medical provider and, when appropriate, other mental health providers.

Essential Job Functions:

• Supports the behavioral and physical health care of patients on an assigned patient caseload. Closely coordinates care with the patient’s medical provider and, when appropriate, other mental health providers.
• Screens and assesses patients for common mental health and substance abuse disorders. Facilitates patient engagement and follow-up care.
• Provides patient education about common mental health and substance abuse disorders and the available treatment options.
• Systematically tracks treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
• Supports psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
• Provides short-term therapy services using evidence-based techniques such as CBT, ACT, DBT skills etc.
• Participates in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicates resulting treatment recommendations to the patient’s medical provider.
• Tracks patient follow up and clinical outcomes using a registry. Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients.
• Facilitates creation of treatment plans and changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized behavioral health care.
• Facilitates referrals for clinically indicated services inside and outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
• Develops and completes relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
• Responsible for maintaining accurate electronic patient records while remaining in compliance with all HIPAA and Confidentiality policies and procedures. Maintain patient charts and records according to established Electronic Health Records guidelines for documentation, billing, coding, and procedures for the range of services delivered within the Collaborative Care model of care.
• Protects and maintains the confidentiality of patient records and information, proprietary information, and personnel information. Completing all clinical documentation clearly, thoroughly, accurately and within timeframes specified by El Rio Health policies and procedures and regulatory authority.
• Attends all applicable behavioral health and medical provider meetings and participate in on-going training activities as needed.
• Participates in in-service/education regarding Quality Improvement.
• Contributes to team effort; and complies with all facility policies and procedures including but not limited to those addressing HIPAA and Compliance and performs other duties as assigned.
• Establishes and builds collaborative relationships with medical/behavioral health teams through shadowing provider visits, familiarization with provider patient panels, provider-directed psychosocial screenings, integrated care medical/behavioral visits and other best practice methods helpful to promoting fully integrated Collaborative Care.
• Provides education to Primary Care teams regarding the latest information and knowledge pertaining to behavioral health assessment and intervention, related behavioral health conditions, safety, and risk assessment, in order to improve patient care and assist with enhancing utilization of Behavioral Health services throughout the organization.
• Maintains a clean, safe, and hygienic work environment in compliance with all Policies and Procedures including but not limited to work areas, workstations, examination rooms, hand washing, infection prevention and control etc. for this position.
• Demonstrates an understanding of and proficiency with the application of all compliance and reporting requirements respective to Joint Commission Certification (JCC) standar
