Case Manager I (Patient Care Services) - Per Diem or PT Available

Description

The Case Manager I will:

• Promote patients' health maintenance and restoration to provide quality, cost-effective outcomes. 
• Initiate a discharge plan to meet immediate post hospital needs, as well as visualizing long term needs, to optimize health maintenance and prevent readmission. 
• Educate patients, family, and staff.
• Collect, analyze, and interpret data to evaluate current processes, and alter practices that don't achieve optimal patient care and customer service. 
• Seek learning opportunities to expand current clinical knowledge.
• Seek learning opportunities to expand cost containment strategies.

Requirements

• Two (2) years' Med/Surg or related experience
• Clinical case management experience (preferred)
• Bachelor of Science in Nursing (BSN) from an accredited school of nursing
• Current RN license in the State of Vermont or mutli-state compact RN license.
