Accreditation Pharmacist Practice Leader

JOB SUMMARY: The Accreditation Pharmacy Practice Leader will serve the Wake Market with a scope to include, but not limited to, state and federal regulations and accreditation standards by regulatory agencies impacting medication use in acute and ambulatory settings, leading the Wake Market Pharmacy Quality Assessment and Performance Improvement (QAPI) Committee to evaluate the effectiveness of the medication management system, and serving as a subject matter expert available to all pharmacy leaders during survey preparation, continual survey readiness and during on-site surveys. This position would partner with the nursing clinical compliance group for the Wake Market.

Enhances the delivery of evidence-based medicine to patients of the organization through designing, implementing and monitoring medication use systems and programs. Assists with the strategic planning and implementation of pharmacy initiatives for the Division of Pharmacy and serves as a subject matter expert.   Additional responsibilities include education of healthcare professionals, representation on medical center committees, and collaboration on department and interdisciplinary initiatives.

What You Will Need:

• Graduate of an accredited School of Pharmacy with a B.S. in Pharmacy or Doctor of Pharmacy (Pharm.D.) is required or an FPGEC certificate from NABP.
• Current state pharmacist license in good standing through North Carolina Board of Pharmacy is required.

Nice To Have (Not Required):

• Pharmacy residency training or equivalent experience or board certification in area of practice preferred.

SKILLS/QUALIFICATIONS:

• Serves as professional resource to service areas, internal customers of organization (e.g. pharmacists, nurses, physicians) and external customers (patients & families). 
• Participates collegially with members of the department of pharmacy and other internal customers to ensure pharmacy representation in the development of medication use management programs including clinical guideline development, critical pathways, disease management, and drug use programs.
• Develops, organizes, implements, and evaluates educational and development programs for medical, nursing, and pharmacy staff members, including precepting and mentoring pharmacy students and residents, to promote optimal medication utilization, positive patient outcomes and cost containment initiatives. 
• Supports team leader in executing initiatives to meet the needs/goals of the division.  
• Actively participates in the development and implementation of performance/quality improvement and patient safety activities.
• Provides accurate teammate communication as needed for appropriate projects, implementations, or initiatives. 
• Collaborates with pharmacy leaders to manage the formulary and the medication-use process. 
• Identifies opportunities for stewardship of high cost or high use medications in area of practice to limit waste, generate revenue, or reduce overall cost of care, executes approved action plans and continuously tracks progress of stewardship initiatives. 
• Collaborates with other healthcare providers to support the Pharmacy and Therapeutics Committee, associated subcommittees, and/or other pharmacy committees by assisting with communication and enforcement of formulary items, completion of formulary addition requests, development of drug monographs/class reviews/order sets, or other tasks as required. 
• Performs quality assurance and performance improvement of the medication use system as appropriate.   
• Mentors, trains, and assists with the performance evaluation of other teammates as assigned.  
• Updates knowledge and skills of self and other health care professionals to maintain, improve and document competency.
